How To Refer a Patient for Pain Management Service

This Questionnaire addresses the length of the patient’s pain experience, the severity of the pain and begins to uncover the attending mental health concerns which often go hand in hand with a prolonged period of pain. We use this method in order to quickly pinpoint where your patient can be best helped.

Once we have the information attached to your referral we are able to make decisions about what steps will work to empower your patient in their Pain Management. Whether more medication is required, or if there are drug-free therapies which may be initiated, Habit Health can help you help your patient with their specific needs. Alternatively, you can fill out the Örebro musculoskeletal pain screening questionnaire then assess the score using the Questionnaire Scoring Guide. If a score of > 50 is achieved, complete the ACC6273 Provider Referral for Pain Management Form and email both documents to referrals@habit.co.nz
